Registration Form

The trainings take place on one Sunday each month from 8am-3pm. You must attend the full session to get your certificate and be eligible for CEU credits.

The cost of the training is $150.00 (plus a $25 shipping fee for hard copy of notebook). Invoices will be sent to the email you provide. Registration fee must be paid 14 days prior to the session.

Note: The requirement for certification as a Certified Dementia Practitioner (CDP) is attending the 8-hour in-person or online Alzheimer’s Disease and Dementia Care Seminar and receiving a Certificate of Attendance. You then apply to the NCCDP for Certification. In addition to the course cost, there is an application fee /certification fee charged by the NCCDP. Please see NCCDP.ORG for more information.
